What you need to know about applications and billing
When is approval required?
- No approval required for care levels 3–5
- Outpatient treatments with care levels 1–2: approval required
- Outpatient surgeries: prescription and approval required
- For regular treatments, approval is always required
How do I get approval?
- Get a prescription from your treating physician (Form 4)
- Submit the application to your health insurance
- Wait for approval (usually within a few days)
- For regular rides: long-term prescription possible (valid up to one year)
- Carry the medical transport form for every ride
- If exempt: present the exemption certificate to the driver
Who covers the costs?
- Health insurers cover costs after approval or prescription
- Cost coverage by the health insurer is provided
- Co-pay: 10% of ride cost (min €5, max €10 per ride)
- If exempt, the co-pay is waived (exemption valid across insurers)
- Accident injuries: employer liability insurance instead of health insurance
- Exemption applies regardless of care level

Outpatient surgeries & aftercare: Prescription and approval required.
Outpatient rides: Approval must be obtained from your health insurer before the first ride if you have care levels 1–2. For care levels 3–5, no approval is required. For regular treatments, approval is always required.
Exemption from co-payments: If you are exempt from co-payments (e.g. by reaching the burden limit), you must present your exemption certificate to the driver. The co-pay is then waived. The exemption is valid across insurers and regardless of care level.

If you do not yet have a care level but believe you need one, contact your care fund (located at your health insurer). They will send a Medical Service assessor (MD) to your home to determine your care needs.
